Below you'll find four different ways to submit a slip contract. For the online option, please follow the instructions carefully.

Option 1.) Download the Slip Contract here and return it via fax at 606.636.6872

Option 3.) Download the Slip Contract here and delivery it in person to our Ship Store.

Option 4.) Fill out our interactive form here, and follow the instructions below:

1.) Ensure you have Adobe Reader. If you do not, you can download it free from the link at the bottom of this page.

2.) Fill out the form, ensuring that all fields that pertain to your request are filled out.

3.) If you want to save a copy for yourself, click the Save icon at the top of the PDF form, and save it to your computer.

4.) If you use Microsoft Outlook as your email client, you may simply click Submit By Email at the top of the form. Everything that is needed will be filled out for you, as well as the filled out PDF form attached. You can then send this email at your convenience.

5.) If you do not have Microsoft Outlook, your mileage may vary with the Submit By Email button. Please save the completed pdf, and attach it via your preferred email provider to an email. Send this email to danielle@leesfordmarina.com with the subject 2009 Slip Contract.
